Advanced Healthcare for Women & Children is located in Upland, California, and specializes in obstetrics and gynecology for women of all ages in the Inland Empire. We have extensive knowledge in providing excellent healthcare to women for a variety of their healthcare needs. Advanced Healthcare for Women & Children has prenatal care that is exceptional and offers healthy pregnancy and prenatal classes, 3d4d ultrasounds, and delivery at Pomona Valley Hospital and San Antonio Regional Hospital. Advanced Healthcare for Women & Children is honored to provide quality healthcare to women in the community and is located in Upland, CA.
Appointment Scheduling
Reach out to our office between 9:00 am and 5:00 pm, Monday through Friday, to book your appointment. Our team will inquire about your visit's specifics to ensure the best appointment for you.
Cancellation & Rescheduling
If you need to change your appointment, kindly contact us at your earliest convenience, ideally by 4 pm on the prior business day, so we can offer your time to another patient.
Late Appointments
Punctuality is valued at our clinic. We strive to maintain efficient schedules for both patients and providers. If you run late, we'll make every effort to accommodate you, but please be prepared for potential delays or rescheduling.
Urgent Medical Needs:
We offer same-day appointments for urgent medical concerns. An on-call healthcare provider is available outside office hours. For non-urgent matters, please refrain from using the emergency line.
In Case of Emergency:
If you encounter a life-threatening emergency, dial 911 immediately.
Pregnancy Disability Forms
Maternity Leave Details:
Contact your workplace's Human Resources Department to learn about your maternity leave options. In California, you may qualify for "Paid Family Leave" (PFL) to extend your paid time beyond the standard 6-8 weeks. Larger employers (50+ employees) must offer additional unpaid parental leave. Feel free to reach out to our office for more information.
Applying for Pregnancy Disability Leave:
Obtain pregnancy disability leave forms from your workplace's HR department. Complete the patient sections and bring them, along with clear mailing instructions, to our office. Don't forget a medical records release form for us to release your medical information. Our nurses will finalize the forms and send them to the appropriate address.
Medical Records & Prescriptions
Medical Records
You are entitled to copies of your medical records, whether for yourself or another medical provider. You will need to sign a medical records release authorization form, which we can give you. Our office will then provide a copy of your records in a timely manner.
Prescriptions
Prescription refills are processed only during our normal office hours. If you would like to order your prescription through a mail order pharmacy, you can obtain a mail order form from your insurance provider’s website.
